Here at Hollybrook we are committed to ensuring your privacy is protected. We treat it with the highest standard of confidentiality. Our policy does not allow us to share or pass on information unless there is informed consent. You will not get any ‘junk mail’ or marketing as a result of sharing any details. Privacy information has been prepared in accordance with the Data Protection Act (1998) and the General Data Protection Regulations (GDPR 2018).

Any information you share with us is controlled by us and we are registered with the Information Commissioners Office (ICO) as data controllers. We only use the information you share with us on the lawful grounds as defined by the General Data Protection Act. All third parties we work with have been carefully chosen and vetoed by us to ensure they comply with our privacy policy.

ACCESSING YOUR MEDICAL RECORDS


Introduction 

 

In accordance with the General Data Protection Regulation (GDPR), patients (data subjects) have the right to access their data and any supplementary information held by Hollybrook Medical Centre and Sinfin Moor Health Centre; this is commonly known as a data Subject Access Request (SAR).

 

Data subjects have a right to receive: 

  • Confirmation that their data is being processed
  • Access to their personal data
  • Access to any other supplementary information held about them.

 

Data controller 

 

For your GP Medical Records the data controller is Hollybrook Medical Centre and Sinfin Moor Health Centre. Should you have any questions relating to accessing your Medical Records please ask at Reception.  

 

Options for access 

 

As of April 2016, Practices have been obliged to allow patients access to their health record online. SystmOnline will enable the patient to view coded information held in their health record.

 

Prior to accessing this information, a SystmOnline application form will need to be completed and given to Reception with a form of ID provided for security purposes. A paper copy of the form is available from Reception and an electronic version on the Practice website.

In addition, you can make a request to be provided with copies of your health record. To do so, a Subject Access Request (SAR) application form will need to be completed and given to Reception with a form of ID provided for security purposes. A paper copy of the form is available from Reception and an electronic version on the Practice website. Patients do not have to pay a fee for copies of their records.

 

Time frame

 

Once the SAR form is submitted, Hollybrook Medical Centre and Sinfin Moor Health Centre will aim to process the request within 28 days; however, this may not always be possible. The maximum time permitted to process SARs is one calendar month. If there is a delay in processing your request, your practice will contact you to explain the reason.

Exemptions 

 

There may be occasions when your GP practice (the data controller) will withhold information kept in the health record, particularly if the disclosure of such information is likely to cause undue stress or harm to you or any other person. 

The data controller is also responsible for removing any 3rd party information that may be held in your record.
